Alivia Care is a nurse-led, home-based care coordination service for patients with an advanced illness, providing complex care management and a support system to enhance the quality of life of a patient and their caregivers wherever they call home. When a patient moves from receiving around-the-clock care in the hospital to being at home, Alivia is there for you. The goal of Alivia Care is to provide care all ways and to care always.
Complex cases of advanced illness typically involve multiple physicians, medication management, and medical protocols. This oftentimes becomes too complex for the patient and family, and the patient returns to the hospital unnecessarily. Your Alivia Care team of nurses, social workers, and chaplains/ spiritual advisors are there to support you. We will work with you to understand and manage your health issues and tailor a care plan according to your preferences. We care all ways, always.
Alivia Care has partnered with Flagler Hospital and the First Coast Health Alliance to offer Flagler Advanced Care Services (FACS), the first and only collaborative effort in Northeast Florida between a hospital and an insurance company for a new care model provided by Alivia Care.
