National PACE Month: September Celebration at The PACE Place
September is recognized as National PACE Month, an annual observance led by the National PACE Association (NPA) to raise awareness about the Program of All-Inclusive Care for the Elderly (PACE). Since 2017, this month-long celebration has highlighted the importance of the PACE model, which helps older adults receive comprehensive care while remaining in their homes and communities.
This year’s theme, “Simplify Your Life: At Your PACE,” underscores how PACE programs streamline care by bringing medical, social, and support services together to enhance quality of life. At The PACE Place in Jacksonville, we are proud to support this mission by providing coordinated, person-centered care that empowers participants and eases the caregiving burden for families.
Located at 5450 Ramona Boulevard, The PACE Place was the first of its kind in North Florida. We serve individuals aged 55 and older who qualify for nursing home-level care but prefer to live independently in their own homes. Our program offers a wide range of services, including primary care, physical, occupational, and speech therapies, meals, counseling, recreational activities, transportation, and home-based support. A dedicated interdisciplinary team works together to develop customized care plans that meet each participant’s medical and personal needs. We are currently accepting new participants and welcome inquiries and referrals. Families and healthcare providers are encouraged to call us for more information.
PACE programs are designed to help seniors live safely at home while receiving the comprehensive care they need. Participants benefit from coordinated services, which reduce the risk of hospitalization and nursing home placement. Everything from doctor visits and medication management to transportation and social engagement is managed through one team, making life simpler and safer for participants and their caregivers.
In honor of National PACE Month, The PACE Place will be hosting a variety of events and initiatives throughout September. These include an open house for current participants, prospective participants, and referral partners, as well as team member spotlights to recognize the dedicated professionals behind our care model. Additionally, we have several appreciation days. We will also launch a social media campaign featuring participant testimonials and educational content and continue outreach efforts in partnership with senior centers and healthcare organizations in the Jacksonville area.
National PACE Month is a time to reflect on the value of compassionate, coordinated care for seniors. At The PACE Place, we are honored to be part of a movement that promotes dignity, independence, and simplicity in aging. We invite our community to join us in celebration, learn more about our work, and discover ways to get involved.
